Skechers Shape Ups for Kids Disturb Parents and Professionals

Skechers Shape Ups shoes for kids has caused quite the controversy since their release, including a petition at change.org to have the line discontinued. Parents and professionals are disturbed that toning shoes are being made for and marketed to elementary school students.

The commercial aimed at young girls seems to be especially concerning to parents. The commercial in question does not specifically say anything about toning, but it does say that these shoes offer “everything a girl could want, looking good, having fun,” with “extra height and bounce,” which, of course, is too good to be true. Parents are concerned that the thin cartoon characters and emphasis on appearance is encouraging unhealthy attitude towards body image in young girls. Parents are also concerned that the commercial contains a shot of boys dressed as junk food following the singer; it certainly is a confusing image. I would be interested to hear what you think this is communicating to young girls?

I was able to consult with several professionals who are concerned that any kind of toning shoe can cause injuries and long-term health problems for both children and adults. Jacqueline M. Sutera, DPM, a surgically trained midtown Manhattan podiatrist and spokeswoman for the American Podiatric Medical Association who was recently on the Dr. Oz show has been reminding parents that “the shoe’s rocker bottom can cause ankle and foot injury especially if the children are running around and very active. We have to remember that children are not small adults. Their growth plates are open, and can potentially be damaged by altering their gait. Also, the growing child’s soft tissues (ligaments, muscles, tendons, etc) are also still maturing and susceptible to injury and damage.

Wendy Willis, MA, FAFS, personal trainer/movement specialist and Fellow of Applied Functional Science from the Gray Institute for Functional Transformation, states that these shoes “will cause long term affects on the rest of the body (usually knee pain, lower back pain, and neck pain),” but “that is only one thing that [she has] a problem with.” She continues to explain that she has many concerns and “these shoes are very disturbing for adults – I am appalled that we would subject children to them also. They limit so much movement – jumping, dodging, lateral movement… Kids are already not moving enough. Legs should be toned by moving in all sorts of ways. NOT ONE!!!”

Parents who are willing to spend up to $100 for toning shoes for their children may need to step back and look at their own motivations and what message purchasing such shoes will communicate to your children. If your child is requesting such shoes, it is an excellent time to assess if your child is susceptible to advertising or if she is having body image concerns. You can say no to shoes that may damage your child’s health, but YES to helping her improve her body image by working together to create healthier meals, getting active as a family, and helping your child find positive personality traits in herself. If you are wearing such shoes yourself, please know that your child will be concerned about toning her legs and buttocks and will want to wear shoes just like you. You may be willing to risk injury to yourself, but are you willing to risk your daughter doing the same to her developing body?

What are children's dosage forms and how to take drugs for children?

Pharmacists often have to answer customer questions about medicines for children. How to take them correctly, what form to choose? All these nuances affect not only the effectiveness of treatment, but also the quality of life of the child and his close environment using dispensers. Well, be sure to have a protective device that excludes the possibility of opening the bottle by a child.

France takes the lead in the production of children's pharmaceutical forms, the USA, Great Britain, Germany are also among the leaders. On the Ukrainian pharmaceutical market, most of the drugs used in pediatrics are represented by foreign manufacturers.

Special organism - increased requirements

A child's body differs significantly from that of an adult in terms of adsorption rate, rate of metabolic processes and features of excretion of chemicals, as well as tissue hydration. In addition, enzyme systems are not yet fully developed in children, there are features regarding heart contractions and respiratory rate. Also, children are more prone to various allergic reactions, do not tolerate any pain, specifically react to an unfamiliar taste. Therefore, the requirements for children's forms of drugs have a number of stringent requirements.

All over the world, patients are divided into age groups. Among children, a group of newborns, children of the first year of life, up to 3 years old, up to 12 and over 12 years old are distinguished.

Pharmaceutical preparations for infants are manufactured sterile. Such medicines must be labeled in some way. Available mainly in the form of drops.

Children of the first year of life can already be prescribed dosage forms in the form of syrups, suspensions, emulsions, as well as solutions, drops, rectal suppositories.

From the age of five, not only liquid dosage forms are used, but also effervescent tablets that dissolve in liquids, as well as powders - they are added to food.

Starting from the age of six, the pharmaceutical range of drugs is expanding with chewable and dissolving tablets in the oral cavity. From the age of twelve, the usual dosage forms for adults are used.

In form and content

In terms of aggregate state, pharmaceuticals for use in pediatric practice can be liquid (syrups, suspensions, emulsions, drops), solid (capsules, tablets), gaseous (sprays, aerosols).

Liquid forms account for 70% of the total number of preparations. A distinctive feature of such preparations intended for taking per os is rapid absorption, ease of dosing and ease of use. Due to the high content of corrigents, they have a more pronounced taste and smell; as a rule, they contain preservatives, stabilizers, co-solvents. However, all these components can cause additional adverse reactions and complicate treatment. Therefore, it must be remembered that preparations for children are a complex therapeutic system in which it is important to take into account potentiation, synergism of action and excretion from the body.

Almost a third of all children's dosage forms are tablets. And although the size of capsules and tablets presented by manufacturers is very different (from 5 mm to 24 mm), according to the observation of both doctors and parents, children are more willing and easy to take small tablets than syrups, which are perceived as classic children's forms. In addition, children with chronic diseases show greater adherence to treatment and are taught from an early age and can take capsules and tablets. Here, probably, the routine of actions and habit matters.

Rectal suppositories are most often prescribed for newborns and children of the first year of life, and also if the child has severe vomiting. The absorption of the active substance during rectal administration occurs as quickly and actively as possible, therefore, if the instructions for use are not followed, there is a threat of toxic complications.

Drops and sprays have their own characteristics. For example, children's sprays differ from adults in that the medicine is sprayed with less pressure. The use of nasal drops for adults can provoke otitis media in children - when sprayed in the nasal passages, excessive pressure is created and part of the mucus from the nasopharynx enters the auditory tube. In addition, children's sprays have a lower concentration of the active substance, say 0.1% for adults and 0.05% for children. Therefore, when they ask to sell just nose drops under such and such a name, it is necessary to clarify for whom they were prescribed.

If we talk about ointments, in this case it is important to remember that active substances are more actively absorbed into the blood through children's skin, so ointments for children and adults have different concentrations.

Dosing devices

Strict adherence to doses is an important rule in the treatment of children, which means that the creation of convenient dispensers for medicines is essential. That is why the world's leading manufacturers are developing special devices that can be used to dose the drug correctly, taking into account the age and weight of the child. As a rule, these are oral dispensers, measuring cups or spoons, pre-filled syringes, dissolvable films, pipettes, aerosol dispensers with a certain pressure.

At the same time, the packaging of medicinal products must be thought out to the smallest detail. So that adults can easily intuitively open it, while this method of opening should not be understandable and accessible to children.

To divide or not to divide

Sometimes tablets and capsules have to be divided into several parts. Most often, such actions are taken for economic gain or in order to achieve the desired dosage. However, some drugs need to be taken only as a whole. If there is no clearly marked risk on the tablet, it has a convex shape, crumbles easily, and it cannot be divided into parts. With uneven separation, the amount of active substance in individual parts will be different, and, therefore, the effect of the treatment will decrease. This rule is especially true for antibacterial agents. In the case of adults, this rule must be remembered when taking antihypertensive drugs and anticoagulants.

Not subject to division into parts of the tablet, which have a special relatively dense coating and capsules. The active substance of such drugs should be gradually released in the gastrointestinal tract and gradually enter the bloodstream. In case of violation of special shells, the effect of the drug changes.

If the drug contains not one, but two or more active substances, when divided, it will lose therapeutic efficacy, since it is not known how much, which substance was obtained to a greater or lesser extent and when this happened.

It is possible to divide into equal parts tablets with clearly marked notches, which contain only one active substance.

Time of taking and compatibility of drugs

When consulting pharmacies, it is important for the first-timer to remember that the absorption of drugs in the digestive tract is affected by the diet, and the time of taking the drugs is also important. So drugs that can irritate the gastrointestinal tract are recommended to be given to the child after or during meals - preferably with the first spoonfuls of food at the peak of nutritional interest. The effectiveness of drugs is significantly affected by the pH of the liquid that is washed down with drugs. Fruit and vegetable juices may reduce the effectiveness of some acid-sensitive antibacterial drugs (penicillin, erythromycin). But dairy products affect the absorption of tetracyclines. All the features of taking drugs are described in the instructions, however, customers should always be reminded of them additionally.

Drugs that affect the digestive process are included in a special group and require careful implementation of the requirements specified in the instructions. For example, antacids - drugs that lower the acidity of gastric juice - should be drunk half an hour before meals. An interval of at least an hour should be between taking antacids, enterosobents and other medicines. Also, the time interval is important to observe in the case of the appointment of antibacterial drugs and probiotics.

  • In order to give the correct form in the correct dosage, it is necessary to clarify the age and weight of the child.
